 Indications:

Severe pain. Relief of dyspnoea of left ventricular failure and pulmonary oedema. Pre-op use.

 Adults:

5—20mg by sc or im inj every 4 hrs as required, adjusted as necessary. Slow iv inj: use ¼—½ im dose.
Patient controlled analgesia, use 1mg/ml soln. Usual loading dose 1—10mg (max 15mg) over 4—5 mins then 1mg on demand every hour with 5—10 min lock out time.

 Children:

Under 1 year, not recommended. Acute pain by sc or im inj every 4 hrs as required: 1—5 years, 2.5—5mg; 6—12 years, 5—10mg. Acute pain by slow iv inj every 4 hrs as required: 100—200 microgram/kg; alternatively, 100—200 microgram/kg by slow iv inj then 20 microgram/kg/hr by continuous iv inf, adjusted according to response. Chronic pain, by sc or im inj every 4 hrs as required: 1—5 years, initially 150—200 microgram/kg; 6—12 years, initially 200 microgram/kg. Adjust as necessary.

 Contraindications:

Respiratory depression, hypoxia, hypercapnia, severe asthma, obstructive airways disease, heart failure secondary to lung disease, acute hepatic disease, paralytic ileus, delayed gastric emptying, acute abdomen, biliary obstruction, following biliary surgery, 24 hrs before cordotomy, acute alcoholism, raised intracranial pressure, head injury, coma, convulsive disorders, phaeochromocytoma, excessive bronchial secretions, ulcerative colitis, biliary and renal tract spasm. Acute diarrhoea associated with antibiotic-induced pseudomembranous colitis or poisoning. Pregnancy, lactation. Sust-release preps: pre-op and 24-hr post-op use.

 Warnings:

Risk of tolerance and dependence; ensure withdrawal strategy in place before starting treatment. Hypothyroidism, renal or hepatic impairment, asthma, intracranial lesions, unexplained loss of consciousness, sleep apnoea, hypotension, hypovolaemia, urinary or biliary tract disease, pancreatitis, obstructive or inflammatory bowel disease, constipation, prostatic hypertrophy, adrenocortical insufficiency, reduced respiratory reserve, shock, sickle cell disease, surgery, history of epilepsy, hypopituitarism, diabetes, myasthenia gravis, arrhythmias, excessive obesity, severe cardiac failure, post total joint arthroplasty. History of substance misuse or psychiatric disorders. Risk of decreased sex hormones and increased prolactin with long-term use. Withdraw gradually. Elderly.

 Interactions:

MAOIs, TCADs, CNS depressants, cimetidine, ranitidine, opioid antagonists or partial agonists, rifampicin, muscle relaxants, antihypertensives, gabapentin, voriconazole, ritonavir, mexiletine, antihistamines, dopaminergics, antiemetics, alcohol, antipsychotics, dexamfetamine, diuretics, anticholinergics, disulfiram, ciprofloxacin, domperidone, metoclopramide, antidiarrhoeal and antiperistaltic agents, cough suppressants, antiplatelets, sodium oxybate.

 Adverse Effects:

GI upset, dry mouth, headache, dizziness, drowsiness, cognitive disorders, mood change, confusion, insomnia, changes in activity, dysgeusia, miosis, pruritus, rash, sweating, involuntary muscle contractions, anorexia, urinary retention, asthenia, fatigue, malaise. Reports of hyperalgesia.
